Social Worker (H/HHA Coordinaor)

The H/HHA-IHR Coordinator is an advance practice social work coordinator who is primarily responsible for the management, integration and evaluation of these programs at the NCHCS. In addition to these duties, the H/HHA-IHR Coordinator has clinical responsibilities and provides social work services to eligible veterans, either in the context of program management, or as an adjunct social worker in the Community Care Program. The Social Work Coordinator evaluates consults for the Homemaker/Home Health Aide and In-Home Respite programs and assesses the Veterans' eligibility for services based on established criteria such as the Respite Handbook, Purchased Home Health Care Services Handbook, and NCHCS policies pertaining to these services. The coordinator assesses the appropriate services based on the Veteran need, recommendation of VA providers, and the availability of H/HHA services. The SW coordinator reviews the plan of service every 6 months, and/or as often as needed, and maintains regular contact with home health agencies and Veterans in the programs to address problems or issues, with the goal that the service helps support the Veteran in the home setting. The coordinator is aware of federal, state and Joint Commission regulations pertaining to home health agencies, and ensures that agencies follow all pertinent regulations. The coordinator audits agency records as needed to ensure compliance with these regulations. Duties consist of monitoring H/HHA programs, acting as a point person for referrals, admissions and providing oversight of Veterans that are placed in these programs. Work Schedule:

Citizenship - Must be citizen of the United States English language proficiency - Must be proficient in spoken and written English Education - Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet requirement until the school of social work is fully accredited. //www.cswe.org/Accreditation to verify that the social work degree meets accreditation standards for a Master of Social Work.(COPIES OF TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION) Licensure - Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. (COPIES OF LICENSE AND CERTIFICATION MUST BE SUBMITTED WITH APPLICATION) - Must be fully Licensed Clinical Social Worker. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation). Grade Determinations:
Social Worker Program Coordinator, GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
