Clinical Pharmacy Specialist - Home Care Coordinator

Manages pharmacy care within Home Care Coordination program Serves as a link between Geriatrics and Extended Care (GEC) Service and all healthcare disciplines included on the Home Care Coordination (HCC) teams. Approves referrals to the Home Based Pharmacy Care (HBPC) program and other VA staffed in-home programs Collaborates with the HCC Medical Director and other leaders in the VA Healthcare community. Contributes to medical center policy development, planning, decision making, resource management and evaluation that affects the delivery of health care services to patients. Interprets and communicates VA policies to the HCC team, VA and Non-VA providers, and the VHA facility personnel. Participates with the HCC pharmacy team to develop and implement local policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety Evaluates the effectiveness of the HCC program, assures effective/efficient resources are maintained, and assures the program can optimize cost effectiveness (budget) and patient outcomes. Collaborates with the HCC Medical Director to manage human resources, including:
selecting staff, orienting and mentoring staff, promoting staff development and professional growth, and assuring a process for ongoing staff clinical competency. Coordinates trainee education programs. Manages and tracks use of resources including assigned equipment and government vehicles. Tracks referrals for HCC and ensures timely access to HCC program. Manages the Electronic Waiting List for HCC referrals. Collaborates with the Business Office and Fiscal Service to ensure accurate reporting clinical, administrative and workload data. Serves as subject matter expert for HCC and HBPC services such as Home Oxygen Programs, home medical equipment and contracting services to comply with regulatory standards. Evaluates HCC program activities and provides follow up actions as required. Work Schedule:

Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (PharmD)degree. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. In addition to the GS-12 requirements, applicants must have 1 year of experience equivalent to the GS-12 grade level including providing evidence-based pharmaceutical care services through direct care with patients and providing consultations with other providers on their team as well as experience demonstrating the following Knowledge, Skills and Abilities. Knowledge, Skills and Abilities (KSAs):
Pharmacists assigned to this position must demonstrate the following KSAs in their application documents. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Completion of as clinical pharmacy residency or equivalent experience as well as experience in home based primary care or transitions of care programs is preferred but not required. References:

The work requires operating a motor vehicle during day and nighttime and various weather conditions, standing, light carrying and lifting (under 15 pounds); walking, bending, reaching above the shoulder, and ability to distinguish basic colors. The work also involves working around machinery with moving parts and working closely with others. A physical examination prior to placement is required.
